  On September 23, 2026, from 1:00 to 2:00 pm, the Ontario Collaborative for Aging Well (OCAW) will hold a province-wide open forum for Ontario Health Teams, Primary Care Networks, Specialized Geriatric and Seniors’ Mental Health Services and community partners working to support older adults and care partners as part of ongoing the OCAW discussion forum. Co‑facilitated by PGLO and the Centre for Studies in Aging and Health, this discussion forum focuses on shared learning, real-world innovations, and cross‑sector conversations that support older adults and their care partners.
