On September 23, 2026, from 1:00 to 2:00 pm, the Ontario Collaborative for Aging Well (OCAW) held a province-wide open forum for Ontario Health Teams, Primary Care Networks, Specialized Geriatric and Seniors’ Mental Health Services and community partners working to support older adults and care partners to the OCAW discussion forum. Co‑facilitated by PGLO and the Centre for Studies in Aging and Health, this format focuses on shared learning, real-world innovations, and cross‑sector conversations that support older adults and their care partners.

Enabling Aging in Community: What Happens Before Someone Needs Care?
Presentation summary: What does it really take to enable people to age in their communities? This session explores how upstream community design, cross-sector partnerships, and the Age-Friendly Communities framework can help Ontario Health Teams and their partners create the conditions for healthier aging, greater independence, and reduced frailty before people need more care.
 

Our Speaker:
Sarah Webster holds a Master of Health Studies and a B.A. in Gerontology and Psychology. She is Manager of the Centre for Studies in Aging and Health at Queen’s University, and co-Chair of the Ontario Collaborative for Aging Well. Sarah began her career running recreational programming in long-term care. She later transitioned to systems-level work and now has extensive experience in the field of knowledge translation and exchange. Sarah is particularly passionate about age-friendly communities and in her role with the Centre for Studies in Aging and Health has contributed to the growth and sustainability of the Ontario Age-Friendly Communities Outreach Program since its inception in 2015. Sarah has provided knowledge translation expertise to numerous groups including Niagara Connects, Behavioural Supports Ontario, Gestalt Collective and the Seniors Health Knowledge Network, among others.
