Join us on February 25, 2026 from 1:00 to 2:00 pm for the new Ontario Collaborative for Aging Well (OCAW) discussion forum. OCAW has now transformed into a province‑wide open forum for Ontario Health Teams, Primary Care Networks, Specialized Geriatric and Seniors’ Mental Health Services and and community partners working to support older adults and care partners. Co‑facilitated by PGLO and the Centre for Studies in Aging and Health, the new format will focuses on shared learning, real-world innovations, and cross‑sector conversations that support older adults and their care partners.

This month features:

Digital Literacy for Older Adults: Exploring the Enablers and Barriers for this population

Presented by: Dr. Madison Robertson, Research Coordinator, Centre for Studies in Aging and Health
Madison completed her PhD in Health Quality,and holds a master’s degree in kinesiology. She has experience working on and leading national and international multi-disciplinary research projects and currently instructs university level courses on Healthy Aging. In her role, Madison provides support and coordination to research and evaluation activities in aging and health care services for the CSAH team.

Frailty First: Proactive Frailty Screening & Management in the ED

Presented by: Dr. Sabeen Ehsan, Director, Quality and Planning, Seniors Care Network 

Sabeen is internationally trained MD and holds an Executive Master of Health Informatics from the University of Toronto, specializing in data analytics, artificial intelligence, and digital health solutions to enhance service planning and delivery. Sabeen is a forward-thinking strategic leader with a passion for transforming older adult care. With a background in clinical practice and hospital quality management, she brings a global perspective to health system innovation, service design, and performance improvement.

 

Older Adults in Ontario: An Emerging Data Story

Presented by: Dr. Kelly Kay, Executive Director, Provincial Geriatrics Leadership Ontario

Kelly holds a PhD from the University of Toronto, and focuses on health services design for older adults living with complex health conditions. Her work has included the development of competency frameworks in interprofessional geriatric and dementia care, the creation of a provincial asset inventory for geriatric and seniors’ mental health services and the design and implementation of clinical geriatrics models.
