Ontario Collaborative for Aging Well Discussion Forum – February 2026
Join us on February 26, 2026 from 1:00 to 2:00 pm for the new Ontario Collaborative for Aging Well (OCAW) discussion forum. OCAW has now transformed into a province‑wide open forum for Ontario Health Teams, Primary Care Networks, Specialized Geriatric Services and and community partners working to support older adults and care partners. Co‑facilitated by PGLO and the Centre for Studies in Aging and Health, the new format will focuses on shared learning, real-world innovations, and cross‑sector conversations that support older adults and their care partners.

From Frailty to Resilience Webinar Series: Data-Driven Decision-Making in Rehab Care for Older Adults

Ontario Collaborative for Aging Well Relaunches as an Open Forum for OHTs and PCNs
Beginning in 2026, OCAW will transition to a province‑wide open forum designed to bring together Ontario Health Teams (OHTs), Primary Care Networks (PCNs), Specialized Geriatric and Seniors' Mental Health Services (SGS & SMH) and community partners working to support older adults and care partners. Co‑facilitated by PGLO and the Centre for Studies in Aging and Health (CSAH), the revitalized OCAW forum will create a flexible, accessible space for knowledge sharing, collective problem‑solving, and highlighting emerging practices across the province.
