On June 17, 2026, from 1:00 to 2:00 pm, the Ontario Collaborative for Aging Well (OCAW) invites Ontario Health Teams, Primary Care Networks, Specialized Geriatric and Seniors’ Mental Health Services and community partners working to support older adults and care partners to the OCAW discussion forum. Co‑facilitated by PGLO and the Centre for Studies in Aging and Health, this format focuses on shared learning, real-world innovations, and cross‑sector conversations that support older adults and their care partners.

This month (June), which is Seniors’ Month, we feature:


Frailty to Resilience: Rehabilitative Care Alliance (RCA) Rehab Access Pathway

Learning Objectives:

  • Review the RCA Rehab Access Pathway to guide referrals for older adults with a change from baseline function to the appropriate level of rehabilitative care.
  • Explore one regional (OH Toronto) referral process example of direct access from Emergency Departments to Inpatient Rehab.

Presented by: Gabrielle Sadler, MSc, Project Manager, RCA

Gabrielle Sadler is an innovative health care leader with a passion for ensuring that people receive the best rehabilitative care across the continuum of care. Her experience as a physiotherapist and health care administrator spans both hospital and community settings, including transdisciplinary home programming, acute and critical care, inpatient rehabilitative care, stroke care, geriatrics and palliative care. Gabrielle is a certified LEAN Six Sigma Black Belt in Health Care Delivery, complementing her current role as a project manager with the Rehabilitative Care Alliance, with a mandate is to strengthen and standardize rehabilitative care across Ontario.

Dr. Heather MacLeod is the Director, Programs and Partnerships, at Provincial Geriatrics Leadership Ontario (PGLO), where she currently leads the Provincial Common Orientation to the Care of Older Adults, a program that has provided training to more than 1,380 health and social care professionals from over 300 organizations across Ontario.
