On September 23, 2026, from 1:00 to 2:00 pm, the Ontario Collaborative for Aging Well (OCAW) held a province-wide open forum for Ontario Health Teams, Primary Care Networks, Specialized Geriatric and Seniors’ Mental Health Services and community partners working to support older adults and care partners to the OCAW discussion forum. Co‑facilitated by PGLO and the Centre for Studies in Aging and Health, this format focuses on shared learning, real-world innovations, and cross‑sector conversations that support older adults and their care partners.
