On November 3, PGLO hosted a dynamic, virtual, provincial information exchange focused on the evolving landscape of coordinated access and centralized intake in Specialized Geriatric Services (SGS) and Seniors Mental Health (SMH) across Ontario. This summit brought together more than 75 SGS & SMH leaders to discuss the current state of coordinated access and centralized intake and share how their programs are designed and evolving—covering staffing models, processes, and the types of data being collected.
This collaborative dialogue helped to:
- Identify the current state of development of coordinated access and centralized intake practices across the province.
- Understand staffing mixes, workflows, and operational models.
- Explore data collection practices, including what is being gathered, emerging trends, and how programs are adapting in response.
- Identify opportunities for further development, innovation, and integration.
- Discuss challenges faced in implementation and sustainability.
- Examine outcomes being tracked, and how these inform our understanding of system (emergency departments, acute care, community, primary care) needs and service demand.
The presentations (found below) help to deepen understanding of how these practices inform older adult health care system needs, anticipate service demand, and contribute to more responsive and integrated health care.
