A CGMHA is a detailed review of an older adult’s mental, emotional, cognitive, functional, and physical health. The goal is to understand your unique strengths, concerns, and needs to develop a personalized care plan that optimizes mental health, enhances quality of life, and supports well-being. The CGMHA may be carried out in different health care settings including acute care, primary care and community-based care.
