Seniors Care Network, a partner of Provincial Geriatrics Leadership Ontario, recognized need for a standardized approach to frailty (prevention, screening, assessment, management). In response to requests from Ontario Health Team partners, and as part of their work with the Ontario Collaborative for Aging Well, Seniors Care Network conducted a rapid review of frailty screening tools and set out to learn from the practices of other jurisdictions. Their work helps supports the design of a regional approach to frailty care, allowing for local variation to reflect available resources and services.
