Provincial Geriatrics Leadership Ontario (PGLO) coordinates Specialized Geriatric Services (SGS) and Seniors’ Mental Health (SMH) services to advance integrated care for older adults living with complex health conditions, including dementia, frailty and mental health conditions, and their care partners, in Ontario. PGLO is funded by the Ministry of Health.

Our Strategic Directions

The work of PGLO builds and expands on prior work initiated by the former Regional Geriatric Programs of Ontario, and many new initiatives, including (among other activities): planning and capacity building, clinical model development, performance measurement and evaluation supports, implementation guidance, knowledge creation and evidence dissemination, and policy development.
Engagement
PGLO convenes an extensive network of partners through its committee structure. This structure enables more than 130 ongoing contacts with older adults, care partners, clinical geriatric and seniors’ mental health services, primary care, interprofessional teams, Ontario Health Teams, researchers and others. These partners provide support to identify priorities and to action PGLO’s annual implementation plan.

Our Leadership
The overall direction for PGLO has been established by the MOH. Ongoing direction and oversight is provided by a Steering Committee, which includes representatives with lived experience as caregivers and/or older adults, as well as clinicians and health administrators. Key advice is provided by the PGLO Older Adult and Care Partner Advisory Council. The membership of these committees is listed below.
