The Ontario Collaborative for Aging Well (OCAW), a grassroots forum for supporting efforts to manage the care of individuals living with frailty, is entering an exciting new stage of development. Beginning in 2026, OCAW will transition to a province‑wide open forum designed to bring together Ontario Health Teams (OHTs), Primary Care Networks (PCNs), Specialized Geriatric and Seniors’ Mental Health Services (SGS & SMH) and community partners working to support older adults and care partners.
Co‑facilitated by PGLO and the Centre for Studies in Aging and Health (CSAH), the revitalized OCAW forum will create a flexible, accessible space for knowledge sharing, collective problem‑solving, and highlighting emerging practices across the province.
Why this change?
The needs of older adults and care partners are evolving, and so is Ontario’s health system. The shift toward integrated care and collaborative planning at the OHT, PCN, SGS & SMH level presents new opportunities for shared learning and alignment. The evolved OCAW forum will help connect teams working on similar issues and amplify successful approaches.
What to expect
- Recurring open sessions
- Spotlight presentations from OHTs, PCNs, SGS & SMH, researchers, and community organizations
- Facilitated discussion on priority topics such as dementia supports, caregiver well‑being, integrated primary care, transitions in care, digital tools, and aging‑friendly community development
- A shared repository of tools, resources, and case examples
- Opportunities to contribute, ask questions, and shape future forum topics

Participation is open to:
- Ontario Health Teams
- Primary Care Networks
- Regional Geriatric Programs
- Specialized Geriatric Services
- Seniors’ Mental Health Programs
- Community and social service partners
- Clinicians, administrators, researchers, and lived‑experience contributors
- Health planners
There is no membership requirement—participants may join any sessions of interest.
