This guidance document is intended to guide the implementation of frailty screening and management in primary care settings. An accompanying document titled ‘Customizable Templates for Frailty Screening and Management in Primary Care’ has been developed to help providers tailor the recommended practices in accordance to their preferences and service & program availability in their local area/region.
These resource documents are based on the ongoing efforts of the members of the Ontario Collaborative for Aging Well (OCAW); a grass-roots group of local, regional, and provincial partners who work with older adults living with or at risk of frailty.
